Body Contouring

ABDOMINOPLASTY (TUMMY TUCK)

A tight abdomen and a slim, narrowing waist are among the most important signs of youth. The abdominal area is one of the fastest regions to deteriorate after childbirth and rapid weight gain or loss. After childbirth, the abdominal skin loosens, stretches, and bulging occurs in the abdominal muscles and abdomen. After menopause, fat accumulation mainly occurs around the belly and waist.

Abdominoplasty is a cosmetic surgery procedure that includes the removal of excess fat and skin in the middle and lower abdominal regions. During the surgery, the loosened muscle tissue is also addressed and tightened.

Who is a candidate for abdominoplasty?

Full abdominoplasty is often suitable for individuals who have given birth or lost significant weight. If there is excess skin above the navel and when sitting, a fold can be held below the navel with the hand, the patient is a good candidate for full abdominoplasty. If the excess skin is only below the navel, a mini abdominoplasty is performed.

How is full abdominoplasty performed?

In full abdominoplasty, liposuction is first performed and the abdominal tissue is shaped. A long incision is made inside the underwear, including any previous cesarean scar if present. The abdominal skin is lifted up to the ribs, and the navel is detached from the skin. The loosened abdominal muscles are tightened and all excess skin is removed. A new hole is prepared for the navel, and the navel is adapted to its new position.

What precautions should be taken after abdominoplasty?

During the surgery, patients are given anti-embolism stockings, a urinary catheter is inserted, and drains are placed. After surgery, patients wear a corset. Efforts are made to have patients stand on their feet on the first night. Patients are required to walk leaning forward for the first week and then gradually stand upright. Drains are removed 2 to 3 days post-op depending on the amount of fluid. Patients can return to daily activities after 1 week, and all sports activities are allowed after 4 to 6 weeks.

What complications may occur after abdominoplasty?

The most feared complication after surgery is pulmonary embolism. The risk increases in elderly, diabetic, and smoking patients. Patients with excessive weight are advised to lose weight before surgery. Other complications include wound healing problems and tissue loss. Excess skin removal, smoking, and early sports activities increase these risks. Some patients may develop fluid accumulation under the skin after a while, which is drained with a syringe. Pregnancy is possible after abdominoplasty, but if pregnancy occurs, the abdominal wall may loosen and sag again.

THIGH LIFT

Looseness, irregularity, and waviness on the inner thighs are among the most disturbing problems for women. Aging, weight changes, and childbirth accelerate loosening in the inner thighs. Loose skin can also cause irritation by rubbing. In thigh lift surgery, excess fat and skin on the upper and inner thighs are surgically removed. The effect is mainly limited to the inner thigh.

Who is a candidate for thigh lift?

Thigh lift is generally performed on women over 40 years old. Patients with excess skin on the inner thighs due to weight changes or childbirth are suitable candidates.

How is thigh lift surgery performed?

Thigh lift is performed under general anesthesia in a hospital setting. Usually, liposuction is done before thigh lift to remove fat. In patients without excess fat, liposuction may not be needed. Excess skin is removed with a vertical incision, which is not visible when the patient stands.

What precautions should be taken after thigh lift?

Patients stay in the hospital for one night after surgery. Drains are used and removed within 1-2 days depending on fluid amount. All stitches are absorbable, so no removal is necessary. A special corset is worn for 2 weeks. Patients should avoid standing or walking excessively for 3 weeks. All activities are allowed after 6 weeks.

What complications may occur after thigh lift?

With horizontal incisions, scars may widen or move downward over time. When the patient spreads legs, the scar may pull the labia outward. Therefore, vertical incision technique is preferred. Vertical scars are not visible from the front and fade after 6 months but do not disappear completely.

ARM LIFT (BRAZIOPLASTY)

The skin of the upper arm (triceps area) may sag due to aging and weight changes. This causes discomfort especially in summer with short-sleeved clothing. Arm lift aims to remove loosened skin and excess fat, especially above the elbow, mainly in female patients.

Who is a candidate for arm lift?

Usually performed on patients over 40 who have lost significant weight. Some skin types may require earlier surgery.

How is arm lift surgery performed?

Performed under general anesthesia in a hospital. Excess skin and fat areas are marked before surgery. Liposuction is done first to remove excess fat. Loose skin is excised with an L-shaped incision from the elbow to the armpit.

What precautions should be taken after arm lift?

Patients stay one night in the hospital and are discharged the next day. Drains, if used, are removed after 1-2 days. Absorbable stitches eliminate the need for removal. Patients should avoid excessive arm movements for 3 weeks. After 3 weeks, sports except for arm movements are allowed. All activities are permitted after 6 weeks.

What complications may occur after arm lift?

Besides scars, major problems include insufficient tissue removal, contour irregularities, and excessive scar formation (keloids).

GYNECOMASTIA

Gynecomastia is male breast enlargement resembling female breasts. It is seen in nearly half of the male population. Causes include physiological changes, medications, cirrhosis, hormonal deficiencies, congenital issues, testicular problems, thyroid disorders, obesity, and kidney problems. The cause must be understood before treatment. Hormonal issues require evaluation and treatment by an endocrinologist. Gynecomastia during adolescence usually resolves after puberty. Others can be treated surgically.

What to consider before gynecomastia surgery?

As with all surgeries, smoking and alcohol should be stopped 3 weeks before. Aspirin and blood thinners must be stopped at least 1 week prior under medical supervision.

How is gynecomastia surgery performed?

Under general anesthesia in the operating room, the surgery lasts about 1-2 hours. In most cases, liposuction alone is sufficient. If breast tissue is dense, ultrasonic liposuction (VASER) may be used. If liposuction alone is insufficient, glandular tissue is removed through a small incision around the nipple. If excess skin is present, an incision is made around the nipple to remove it.

What precautions after gynecomastia surgery?

Patients are mobilized and walking the evening of surgery and may be discharged the same day if no issues arise. Liposuction leaves almost no scars. Absorbable stitches are used. A special corset is worn for 2 weeks. Sports are avoided for 3 weeks, with all activities allowed after 6 weeks. Swelling, bruising, and firmness in the breasts may occur but resolve within weeks.

What complications may occur after gynecomastia surgery?

High satisfaction rates are reported. The most common issue is insufficient reduction, which may require revision surgery. Contour irregularities and dimpling may occur but are minimized with good planning and technique.

Frequently Asked Questions

Body contouring refers to surgical or non-surgical procedures aimed at reducing unwanted fat and skin accumulations on the body to achieve a more aesthetic appearance. These procedures may include liposuction, abdominoplasty (tummy tuck), breast aesthetics, and arm and thigh contouring.

Body contouring procedures may be suitable for individuals who have localized fat and skin accumulations in certain areas of the body and wish to achieve a fitter appearance. However, they may not be suitable for everyone, so a detailed evaluation with a plastic surgeon before the procedure is important.

The duration and recovery process of body contouring procedures vary depending on the specific procedure performed and the patient’s individual condition. Generally, the procedures last a few hours, and the recovery period can take several weeks or months.

After body contouring procedures, a firmer and more contoured body appearance is generally achieved. However, final results become noticeable within a few weeks or months after the procedure. It is important to consult with a plastic surgeon for more information and to evaluate the outcomes.

Body contouring procedures can be performed together with proper planning between the physician and the patient according to the patient’s needs. In patients with significant weight loss and who require multiple surgeries, the correct planning allows the minimum number of operations to be performed in the most appropriate way.
